  Please note the following:

  1. Completed application forms MUST be accompanied by the full amount of the fee, the required certificates and three signed passport photographs. Applications received without these items will be returned immediately.

    We have provided a link to the application form, which is in WORD format for you to download and fill out. When you click on the below link, the form will appear as a WORD document which you can then print out.

  2. Complete applications should be sent to: Faculty of Radiologists, Royal College of Surgeons in Ireland, 123 St. Stephens Green, Dublin 2, Ireland. Tel: 402 2139, Fax No: 402 2466 and email address: radiology@rcsi.ie

  3. Fees are payable in Euro currency, i.e. €, bank draft made payable to Royal College of Surgeons in Ireland. Refunds on Sterling drafts will not be made

  4. Applications should not arrive NO LATER than the closing date stated. Candidates are advised to make early application, as receipt of applications before the last date of the entry DOES NOT guarantee a place on the examination.

  5. Any candidates wishing to cancel or transfer an application either before or after the closing date will forfeit their fee in FULL. A refund, less 20%, will be made only on the basis of ill-health (medical certificate required) or inability to obtain a visa for entry to the Republic of Ireland (letter of proof required).

  6. Candidates are advised to make arrangements for visas (if appropriate) before closing date for entry to the examination. If there is no Irish Embassy in your country you should contact: Visa Section, Department of Foreign Affairs, 80 St. Stephen's Green, Dublin 2 for advice Tel: 00 353 1 478 0822. It may take up to three months to obtain a visa.

    Please note, the college will issue a letter for visa purposes, stating you have applied for the examination ONLY when a completed applications and FULL FEE has been received by the College. The Examinations Office is not involved with any stage of a visa application and can not enter into discussions with the Department of Foreign Affairs on any candidate's behalf.

  7. Any requests for exemptions, transfers or cancellations should be made IN WRITING ONLY to the Examinations Office including appropriate documentation
